Registering with Bridge House Medical Practice

We welcome applications from all those living in the practice area (see below) and we aim to give top quality care to you, whatever your age, gender, ethnicity or lifestyle.


Practice Catchment Area

Please telephone the surgery to ensure you are living within our catchment area.

Our practice catchment area covers most of the postcode area of N4.

How To Register As A Patient

We can only accept patients from our practice area, the receptionist will check to see if your address falls in the practice area. We can register new patients at any time during our opening hours.

If you would like to register with our practice, please bring your medical card to the surgery if possible. Also bring some photo ID and proof of address. 

I do not have my Medical Card?

If you do not have your medical card, you will need to complete form GMS1, you can do this when you visit, or download a copy from the forms section on this page.

This may arise if you cannot find your cards or if you do not have any e.g. if you have recently arrived in the country. This is less preferable since it may take longer to trace your medical records and you will have to fill in more paper work, so please find your medical cards if you can.

New Patients Check

We ask all new patient to complete a short health questionnaire and make an appointment for a brief health check.

You will be registered to the practice rather than to a specific doctor, which means you have the option of seeing any of the Doctors at the practice.

Temporary Residents

The Doctors are happy to see patients who are temporarily resident in the practice catchment area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.

To register you will need to bring your passport and complete our Temporary Resident Registration form which is available from reception, or can be downloaded from the forms section on this page.
